Communication you can count on

Care matched to the need

Assisted living, personal care, or respite — we'll talk through the right level of care in one call.

Residents keep their own physicians; our physician directs the quality of the care around them.

Assisted living

Ongoing support for older adults who want their independence with a hand during the day.

Personal care

More hands-on daily help with bathing, dressing, mobility, and medication management.

Respite care

A short stay, from a few days to a couple of weeks — a practical step-down after a hospital stay, or coverage while a family caregiver is away.

How a referral works

Four steps, one phone call to start.

1

Call us

Reach the community directly at (229) 251-2201 during office hours, 9 AM–5 PM. Please share patient details by phone rather than the web form.

2

We confirm fit & availability

Availability varies by home, so the earlier you reach out, the more options your patient has. We'll talk through the level of care that fits.

3

Tour & move-in

Families are welcome to tour. We coordinate timing around the discharge date and can arrange statewide transportation to the community for the new resident.

4

Ongoing updates

24/7 on-site nursing, physician-directed daily quality rounds, and same-day communication when anything changes.

Questions from referring partners

What discharge planners ask us

Do you accept referrals from hospitals, case managers, and social workers?

Yes. We work directly with discharge planners, hospital case managers, and social workers. Call the community at (229) 251-2201, 9 AM–5 PM daily, and you'll typically hear back within 30 minutes during business hours.

How quickly can you accept a new placement?

Availability varies by home, so the earlier you reach out the more options your patient has. Call us and we'll confirm the right level of care and current availability, and coordinate the move-in timing around the discharge date.

Do you provide statewide transportation to assisted living?

Yes. Transportation to The Orchard is available for new residents moving in from anywhere in Georgia. Call admissions at (229) 251-2201 to confirm availability and coordinate transportation around the discharge or move-in date.

What levels of care do you provide?

Assisted living supports independence with help during the day. Memory care adds cognitive support, familiar routines, and safety-focused care. Personal care provides more hands-on help with daily activities. Respite care is a short stay for caregiver relief, recovery, or a transition.

How do I confirm payment options and service area?

Call the admissions team at (229) 251-2201 to confirm current payment options, availability, transportation needs, and whether the community can safely support the prospective resident.

What does physician-directed quality oversight mean for my patient?

Nursing staff are on site 24/7. Our Director of Resident Care Quality is a licensed physician who is on call around the clock and on site most weekdays. He directs safety monitoring, staff training, and daily quality rounds across all six homes. Residents continue to use their own personal physicians for medical care.

How do you keep referring partners informed?

We provide same-day communication when anything changes. With the resident's consent, referring partners are kept in the loop alongside the family.

Can I share patient health information through the website?

Please don't send protected health information (PHI) through the web form or by email. Share your patient's clinical details with us by phone at (229) 251-2201 — the form is only so we can reach you.
